To register at Elmwood Family Doctors as a new patient, you will need to:
1) Check to see if you live within the practice catchment area, but please be aware that if you are close to the boundary you may need to check with the practice manager.
Only patients that live within the practice catchment area will be accepted as a new patient.
2) Register online, or alternatively you can download and complete our Patient Registration Pack. The form can also be picked up at our reception desks.
Once completed, please return the forms along with 2 proofs of identity; one must be photographic and one must show your current address. Utility bills or bank statements must be no more than 3 months old.
3) Book your New Patient Health Check (if you have registered online, a receptionist will contact you to arrange this appointment). Please bring a urine sample with you to the appointment. Please note that you will need to see a GP / Nurse if you are on repeat medication. It can take a while to get your full medical records from your previous GP, hence it is best if you make an appointment for an initial assessment and we can then make sure you get all your medicines and checks on time.
