Who is eligible this year?

You may be offered a COVID-19 vaccine in spring if you:

  • live in a care home for older adults
  • are aged 75 or over
  • are aged 6 months to 74 years and have a weakened immune system because of a health condition or treatment

If we have your mobile telephone number, you should of received an SMS message from us which includes a link. Please click on the link to book your appointment online. This is the quickest way to book. If you are unsure how to book online, please watch our video which takes you through it step-by-step.

Booking using the link means that other patients calling for an urgent medical need can get through, and it helps us to manage the high volume of calls we are receiving this year.

If raining please bring an umbrella in case you might be required to wait outside, queues may stretch out into the car park.

Please use the toilet at home before you set off.

We will be using a one way system through the building. Staff will guide patients on where to go.

There will be plenty of staff available on the day to assist you, so please don’t hesitate to ask if you need help in any way

Nurse-led Clinics

Long Term Condition Clinics

Regular monitoring and management are offered to patients who suffer from coronary heart disease, asthma, COPD (chronic obstructive pulmonary disease), hypertension, CKD and diabetes. You will be asked to attend an annual review appointment, usually around your birthday, but our practice nurses will often see patients with these conditions on a more frequent basis to manage them under protocols laid down by the medical team. NHS health checks

We offer health checks for people aged 40 to 74 who do not have any of the following:

  • Diabetes
  • Ischaemic Heart Disease
  • Hypertension
  • History of Stroke / TIA
  • Peripheral Vascular Disease
  • Chronic Kidney Disease (level 3-5)
  • Medication for Hyperlipidemia

You will need to complete a questionnaire, book a blood test, followed by a health check appointment with the Healthcare Assistant, usually about a week later.  Please ask our receptionists for further details.

Travel

Our nursing team can administer the following vaccinations for travel, and these are free of charge;

  • Diphtheria
  • Typhoid
  • MMR (Measles, Mumps & Rubella)
  • Hepatitis A
  • Cholera

These should be given at least 2 weeks before the date of travel in order to gain full immunity from them.

Patients should complete a self-assessment via the NaTHNaC travel website, and check the Foreign travel advice available on GOV.uk before travel. If on your self-assessment, it recommends other travel vaccinations not listed above, these are available at a private travel clinic (google ‘travel clinics near me’).

Long Term Condition Clinics

What is a Long Term Condition Review?

A Long Term Condition review is a type of health check for people with Long Term Conditions. These conditions include: 

  • Diabetes
  • High blood pressure
  • Kidney disease
  • Heart disease
  • Asthma
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Rheumatoid arthritis
  • Learning disabilities
  • Dementia
  • Severe mental health problems

If you have one of these conditions then you will be invited for an annual review or more frequently if necessary. It is vital that you attend all of your appointments so that we can ensure your treatments are still appropriate and safe.

What will happen?

A member of the reception team will contact you around your birth month, either by SMS appointment invitation link or phone call, to invite you to attend your annual review.

The first part of this is with a healthcare assistant. At this appointment we will take a blood sample, check your blood pressure, height, weight and update other health data. 

They will also perform a foot & leg check (if diabetic) and discuss your eating regime and lifestyle.

Your results will be reviewed by a member of our clinical team. Depending on your results you may not need a further review and we will update your medications accordingly. 

If we need to discuss your results and treatment with you, you will be invited to book a follow up appointment with either a nurse, pharmacist or advanced practitioner. We will contact you by SMS appointment invitation link or phone call to book this the week after your initial appointment with the healthcare assistant.

Family Planning

A full range of contraceptive services and advice is available at Elmwood Family Doctors. Contact reception to arrange an appropriate appointment.

Our in-house Vasectomy service is a free NHS service available to anyone who is registered with a GP in Kirklees or Calderdale. Your doctor can refer you to us for this procedure using the NHS e-Referral System or via a letter.

We also provide contraceptive implant insertion and removals.

Kirklees Sexual Health Clinic provide all forms of Emergency Contraception including tablets and emergency coil – a full list of services and clinic details can be found on their website: www.locala.org.uk/services/sexual-health.

Emergency tablets are available from the pharmacy or at an appointment with the doctor.

Huddersfield Contraception ClinicMSI is working in partnership with Locala to provide Long-acting reversible contraceptive (LARC) services to women living in or registered with a GP in Huddersfield. Medical Reports / Forms

We understand you might need to get some forms signed by your doctor for various things like HGV licence application, insurance reports, claim forms etc. We are able to do these for you but please ensure that you inform the receptionist clearly what it is you want, as they will be able to advise you better on how to get forms completed. Some forms require you to be seen by a doctor / nurse and sometimes certain tests are needed as well.

We use iGPR Technologies Limited (iGPR) to assist us with responding to report requests relating to your patient data, such as subject access requests that you submit to us (or that someone acting on your behalf submits to us) and report requests that insurers submit to us under the Access to Medical Records Act 1988 in relation to a life insurance policy that you hold or that you are applying for. iGPR manages the reporting process for us by reviewing and responding to requests in accordance with our instructions and all applicable laws, including UK data protection laws.

Patient Requirements

Please continue to submit your requests for medical reports to Elmwood and we will arrange completion by either one of our clinicians or iGPR.

Please make sure you allow adequate time for the forms to be completed as some of them take time in view of looking into your past records or needing more time to complete the forms. Also when coming in to see a doctor / nurse please ensure you have already filled in the sections which need to be filled by yourself.

You might need to pay for completion of some forms / reports as they are outside the domain of NHS Work. Charges vary depending on the type of forms and time needed to complete these. We follow the rates set or advised nationally by the British Medical Association (BMA).

Why GPs sometimes charge fees

It is important to understand that many GPs are not employed by the NHS. They are self-employed and they have to cover their costs – staff, buildings, heating, lighting, etc – in the same way as any small business. NHS England pays GPs for NHS work, but not for non-NHS work. The fees charged by GPs contribute towards covering their additional costs.

What is covered by the NHS and what is not?

The Government’s contract with GPs covers medical services to NHS patients, including the provision of ongoing medical treatment. In recent years, however, more and more organisations have been involving doctors in a whole range of non-medical work. Sometimes the only reason that GPs are asked is because they are in a position of trust in the community, or because an insurance company or employer wants to ensure that information provided to them is true and accurate.

Examples of non-NHS services for which the practice can charge their own NHS patients:

  • accident or sickness certificates for insurance purposes
  • school fee and holiday insurance certificates
  • reports for health clubs to certify that patients are fit to exercise
  • Examination and reports for renewal of HGV/PSV licences
  • Some holiday vaccinations are not available on the NHS and will be charged

Examples of non-NHS services for which the practice can charge other institutions:

  • life assurance and income protection reports for insurance companies
  • reports for the Department for Work and Pensions (DWP) in connection with disability living allowance and attendance allowance
  • medical reports for local authorities in connection with adoption and fostering

Does the practice have to do non-NHS work for their patients?

With certain limited exceptions, for example a GP confirming that one of their patients is not fit for jury service, GPs do not have to carry out non-NHS work on behalf of their patients. Whilst GPs will always attempt to assist their patients with the completion of forms, for example for insurance purposes, they are not required to do such non-NHS work.

Is it true that the BMA sets fees for non-NHS work?

The BMA suggests fees that practices may charge their patients for non-NHS work (i.e. work not covered under their contract with the NHS) in order to help GPs set their own professional fees. However, the fees suggested by them are intended for guidance only; they are not recommendations and a doctor is not obliged to charge the rates we suggest.

Why does it sometimes take my GP a long time to complete my form?

Time spent completing forms and preparing reports takes the GP away from the medical care of his or her patients. Sometimes, the GP who has seen you more than others in the Practice and therefore is best qualified to complete your form, may be away when you bring your form in. This is why it is necessary to allow for up to 2 weeks before a form may be completed.

I only need the doctor’s signature – what is the problem?

When a doctor signs a certificate or completes a report, it is a condition of remaining on the Medical Register that they only sign what they know to be true. In order to complete even the simplest of forms, therefore, the doctor might have to check the patient’s entire medical record. Carelessness or an inaccurate report can have serious consequences for the doctor with the General Medical Council (the doctors’ regulatory body) or even the Police.

What will I be charged?

The BMA recommends that GPs tell patients in advance if they will be charged, and what the fee will be. Ask Reception to check for you what a fee will be.

What can I do to help?

  • Not all documents need a signature by a doctor, for example passport applications. You can ask another person in a position of trust to sign such documents free of charge.
  • If you have several forms requiring completion, present them all at once and ask your GP if he or she is prepared to complete them at the same time to speed up the process.
  • Do not expect your GP to process forms overnight. Urgent requests may mean that a doctor has to make special arrangements to process the form quickly, and this may cost more.

Community Midwives

Community midwives and maternity support workers deliver your antenatal and postnatal care in the community and are on call for homebirths and to support the Birth Centres. Community midwifery is usually your first contact when your pregnancy is confirmed.

Counselling and Therapy

You can refer yourself to NHS Talking Therapies (previously known as Improving Access to Psychological Therapies) who provide a choice of services for people experiencing common mental health problems such as anxiety, depression, stress, panic and obsessive compulsive disorder. Alcohol and Drug support

Your doctor can refer you for support to help with problematic drinking or drug use, or you can refer yourself, to the Drug and Alcohol Service in Kirklees.

Diabetic Retinopathy

Diabetic patients are contacted to attend the Diabetic eye screening clinic.

Antenatal Clinics

Run by the Community Midwives every Tuesday by appointment or by telephoning the surgery.  At other times the Community Midwives may be contacted by telephoning 01484 343356.

Warfarin Clinics

Elmwood runs clinics for patients to have their warfarin levels controlled. This Anticoagulation service has the ability to test a patient’s INR (International Normailised Ratio, or blood clotting time) and alter their dosage of warfarin accordingly.

Surgical Operations

Minor Surgery

The doctors perform minor surgical procedures in the surgery. Minor ops clinics are run by Dr Frankland and Dr Sharma. If you wish to discuss having a skin lesion removed then please make an appointment with any of the above doctors and they will be happy to advise you.

Family Planning

If a patient requires an appointment with a consultant, the practice will use the NHS e-Referral System to book an appointment which is most convenient for the patient.

What is the NHS E-Referral Service?

If you have been referred by your GP for an appointment with a healthcare provider, such as a hospital, you may be able to book your appointment using the NHS e-Referral Service. You can choose the date and time of your appointment and your GP may be able to book your appointment there and then.

However, you have the right to think about your choices, compare different options and book your appointment at a later stage. If so, you’ll be given an appointment request letter, which includes your unique booking reference number, NHS Number and a list of hospitals or clinics to choose from. In addition you’ll be given a password, which allows you to book your appointment via the NHS e-Referral Service either online on the NHS e-Referral System, or over the phone using the NHS e-Referral Service line on 0345 608 8888.

NHS e-Referral Service will also give you the ability to:

  • plan and manage your appointments around any existing appointments, if you are currently undergoing treatment
  • fit your treatment in with your other commitments, at home and at work
  • choose appointments that fit with your carer’s schedule
  • check the status of your referral and change or cancel your appointments easily, either over the phone or online.

There are a number of different services for people who need internet training. 

The best place to start would be your local library.

Learn My Way‘ is a website of free online learning for beginners, helping you develop digital skills. There are simple tips and instructions that can help you to become a more confident internet user.

The ‘Get Safe Online‘ website provides advice and guidance on how to keep yourself and others safe online.

More than 90% of healthcare providers allow you to book your appointment through the NHS e-Referral Service. Where this is not possible, this will be indicated on your Appointment Request letter by a picture of a phone next to the relevant provider. You or your GP will then have to book directly with the hospital or service using the phone number indicated on the letter.
